29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) On or about July 17, 2018, at 10651 NW 88th St, building 7, Apex, Doral, FL, 33178, the employer failed to provide a guardrail system or PFAS for an employee working from a supported scaffold installing wooden panels to a ceiling of the third floor breezeway; thus exposing employees to an approximate 24-foot fall hazard. b) On or about July 17th 2018, at 10651 NW 88th St, building 7, Apex, Doral, FL, 33178, the employer failed to provide a guardrail system or PFAS for an employee working from a supported scaffold on a rooftop installing wooden panels to a roof eave; thus exposing employees to an approximate 36-foot fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. a. On or about July 17, 2018, at 10651 NW 88th St, building 7, Apex, Doral, FL, 33178, the employer failed to provide an employee with training on the fall hazards associated with the supported scaffolds that he was working from to install wooden panels on the ceiling of a third floor breezeway; thus exposing the employee to an approximate 24 foot fall hazard. b. On or about July 17, 2018, at 10651 NW 88th St, building 7, Apex, Doral, FL, 33178, the employer failed to provide an employee with training on the fall hazards associated with the supported scaffolds that he was working from to install wooden panels on the ceiling of a third floor breezeway; thus exposing the employee to an approximate 24 foot fall hazard.
