29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about August 7, 2018, employees operating the Radiac abrasive cut off machines #1, #3, and #4 in the Met Lab  were exposed to hazards created by rotating parts, flying chips and sparks, and/or point of operation and the employer did not have machine guarding in place.    a.  The area around the blade for all three machines was unguarded exposing employees to accidental contact with rotating parts while placing material in the vise.     b.  The actuator for the pneumatic vise was not guarded from accidental contact on all three machines.    c.  The pinch point was not guarded from being caught in the vise while placing material in the vise on all three machines.
