29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about July 10, 2018, employees were using a boom lift attachment to raise a gantry frame section. The boom lift attachment had not been approved by the fork lift manufacturer for use and an employee was injured when the gantry frame section fell as it was being lifted into place.

29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. a. On or about July 10, 2018, employees were using a fork lift attachment to raise a frame section. An employee was injured when the load shifted and the employee was struck by the leg of the frame section.
