OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer utilized damaged storage rack systems, exposing employees to struck-by and crushing hazards:  a) Auria Holmesville, LLC located at 8281 County Road 245, Holmesville, OH 44633: On and preceding August 29, 2022,   employees were exposed to struck by and crushing hazards associated with inadequately maintained racking systems. Employees used forklifts to load and unload large carpet rolls from damaged Steel King SK2000 pallet racks in the storage/reclaim area and roll goods storage area near cell # 7.  The damaged racks were located near operator stations for other machines and in the normal path of travel through the facility.  The loaded Steel King racks had visible damage/defects which included: deflected/bent columns, missing safety locks, bent cross bracing/diagonal bracing, deflected beams.
