29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. Materials or equipment were placed, either permanently or temporarily, within the exit route.     Employer is failing to ensure that exit routes and exit doors were free from obstructions in the following instances:    A) Southwest exit door and associated egress route in the Digester Feed area were blocked by large diameter truck discharge hoses.    B) Exit route and exit/firewall door between Receiving and Main Process area were blocked by a large diameter internal tank storage discharge hose.    These hazards were last observed on or about August 16, 2018.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    Employer is failing to ensure proper working space in front of the CWT A & B Electrical Control Panels in the Polymer Pump area. Side B Polymer Pump unit was located approximately 22 inches in front of the control panels. A zip-stripped electrical cord was also blocking the panel access area. This hazard was last observed on or about October 11, 2018.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv)(a):    Employer is failing to ensure that flexible electrical cords were not used as a substitute for fixed wiring and attached to building surfaces including, but not limited to the following instances:    A) Flexible electrical cords are attached to the polymer pumps in the digester feed end of the facility are wrapped around and zip-stripped to building surfaces and adjacent equipment.    These hazards were last observed on or about October 11, 2018.
