29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Exterior of Building - Roof: Employees engaged in roofing activities were exposed to fall hazards of approximately 50 feet while installing foam on the deck next to a parapet wall 16"-24" tall, on or about 6/9/18.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan.    a) Interior Stairway - 1st Floor to 4th Floor: Employees engaged in roofing activities in the 4th floor were exposed to tripping hazards by utilizing the four story unfiled metal stairway to access the working area, on or about 6/9/18.
