1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years: On or about August 7, 2018, and at time prior thereto, at this precast/prestressed concrete products manufacturing plant, employees were involved in loading and unloading heavy concrete beams, concrete molds, coils of 7 wire cables and supporting stressing jack equipment, and were utilized powered industrial trucks (PITs). The employer did not evaluate their powered industrial trucks operator's performance at least once every three years.

29 CFR 1910.1200(g)(10): The employer did not ensure that, in all cases, the required information was provided for each hazardous chemical, and was readily accessible during each work shift to employees when they were in their work area(s): On or about August 7, 2018, and at time prior thereto, at this precast/prestressed concrete products manufacturing plant, employees were involved in working with and around hazardous chemicals such as, but not limited to portland cement and other silica containing materials. The employees were unaware of location and/or how to access the Safety Data Sheets of those hazardous materials.
