29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 08/06/18, at a 2-story house located at 535 Potomac Ave., Buffalo, NY: Four employees engaged in roofing work on the steep area of the roof on the northwest side of the house were exposed to a falls of approx. 20 ft. because no fall protection system was utilized. All four employees were wearing body harnesses, but none were tethered to a lifeline or any other anchor point. The pitch of that portion of the roof was approx. 6 in 12. NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system alone: a) On or about 08/06/18, at a 2-story house located at 535 Potomac Ave., Buffalo, NY: One employee engaged in roofing work on the low slope area of the roof on the southwest side of the house was exposed to a fall of approx. 18 ft. because no fall protection system was utilized. The employee was not wearing a body harness and no other fall protection system was utilized. The pitch of that portion of the roof was approx. 2 in 12. NO ABATEMENT DOCUMENTATION REQUIRED
