29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 8/9/2018, at the Production area of Plasti-Card Corp. located at 1820 NE 150th St, North Miami, FL. The employer did not conduct annual inspections of the energy control procedure for a die cutting machine (i.e. Shanghai Yawa Printing Machinery Co, Model: PYQ202F, Serial: 94-26) resulting in the exposure of an employee to a caught by hazard while performing servicing and cleaning such as, but not limited to; changing of dies, removal of scrap magnets, etc.

A plastic container with 2351 Fountain Concentrate (containing 2-butooxyethanol, ethylene glycol, magnesium nitrate and N-methyl-2-pyrrolidone), was not labeled with the required information.   b. On or about 8/9/2018, at the Production area of Plasti-Card Corp. located at 1820 NE 150th St, North Miami, FL. A plastic container of UV Rel (containing 2-butoxyethanol, Naphtha, Xylene and Trimethylbenzene), was not labeled with the required information.
