29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a) 600 Church St., Norfolk, VA 23501, General Mail Facility: On or about August 10, 2018, the employer used an approximately 2' T-head three receptacle GFCI extension cord labeled "Do not plug one extension cord into another" attached to a T-head three receptacle extension cord, to power a Euroclean GD 930-H vacuum and a Hilti TE 7-C rotary hammer/drill, exposing employees to potential fire hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
