29 CFR 1910.147(c)(6)(i)(A): The employer did not ensure a periodic inspection of the energy control procedure was performed by an authorized employee other than the one(s) utilizing the procedure, at least annually, to ensure the procedure was being followed. On or about August 8, 2018, and times prior thereto, the employer did not ensure periodic inspections of the lockout/tagout procedures were performed, at least annually for, but not limited to the following machines, to ensure the procedures were being followed. a) Komatsu 60-ton hydraulic press b) Bliss 32-ton punch press c) Bliss 22-ton punch press This exposed employees to caught-in and/or struck-by hazards.
