29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a. On the church steeple roof, on or about August 9, 2018, an employee operating a pneumatic nail gun was not wearing eye protection while exposed to eye injuries from flying debris. Allegiance Roof Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.102(a)(1), which was contained in OSHA inspection number 1290302, citation number 1, item number 1, and was affirmed as a final order on March 14, 2018,, with respect to a workplace located at 643 Lucente Drive, Jacksonville, Florida 32210.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a. On the church steeple roof, on or about August 9, 2018, three (3) employees working from a 4:12 pitch roof were not protected from an approximate 40 foot fall to the lower roof surface by any means of fall protection systems or safety monitoring. b. On the church entryway roof, on or about August 9, 2018, two (2) employees cleaning roofing debris from a flat roof were not protected from an approximate 9 foot, 6 inch, fall to the lower surface (ground) by any means of fall protection systems or safety monitoring. Allegiance Roof Systems, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 1290302, citation number 1, item number 2, and was affirmed as a final order on March 14, 2018,, with respect to a workplace located at 643 Lucente Drive, Jacksonville, Florida 32210. OR IN THE ALTERNATIVE: 29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. At the back side of the church's steeple, on or about August 9, 2018, three (3) employees used an extension ladder which was not extended three (3) feet above the upper landing to access the steeple roof and exposed these employees to a 40-foot fall hazard. b. At the church's front entryway, on or about August 9, 2018, two (2) employees used a Werner brand multi-purpose ladder which was not extended three (3) feet above the upper landing to access the front entryway flat roof and exposed these employees to a 9-foot, 6-inch fall hazard.
