29 CFR 1910.28(b)(8)(ii): For a repair pit, service pit, or assembly pit that was less than 10 feet (3 m) deep, the employer did not apply floor markings at least 6 feet (1.8 m) from the edge of the pit in colors that contrast with the surrounding area; or place a warning line at least 6 feet (1.8 m) from the edge of the pit as well as stanchions that are capable of resisting, without tipping over, a force of at least 16 pounds (71 N) applied horizontally against the stanchion at a height of 30 inches (76 cm); or place a combination of floor markings and warning lines at least 6 feet (1.8 m) from the edge of the pit. a) Pit - On or about August 9, 2018, the employer did not ensure that stanchions with a warning line were put into place at least six feet from the edge of the approximately four foot deep service pit.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Worksite - On or about August 9, 2018, employees are performing maintenance on equipment with multiple sources of energy to include, but not limited to, electrical and hydraulic sources, but the employer did not develop an energy control program with specific procedures, employee training and periodic inspection for the workplace.
