Corner of St. Andrew Blvd. Boca Raton, FL 33434, on a new installed wooden Pole # 769938508, the employer failed to ensure an employee used serviceable outer protector gloves with inner dielectric gloves to maintain a minimum approach distance of 3.71 feet while pulling a live electrical wire of 76.2KV, thus exposing the worker to an electrocution hazard.    NOTE: ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS VIOLATION.
