29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Commercial property at 308 Haddon Ave, in Westmont, NJ, 08108, at the front of the property on the first floor in the south unit: Employees were exposed to electrical shock and fire hazards when they were performing metal roof installation (cutting and measuring) where a 4 outlet gang box that was not mounted and loose Romex wire was that could result in death, which occurred on or about 8/14/18. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Commercial property at 308 Haddon Ave, in Westmont, NJ, 08108, at the front of the property on the roof: Employees were exposed to fall hazards when they performed performing metal roof installation (cutting and measuring) at the top of the roof without any means of fall protection at approximately 15 feet which could result in death and occurred on or about 8/14/18. A. Brooks Roofing Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 977325, citation number 1, item number 1 and was affirmed as a final order on July 1, 2014.
