29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about August 7, 2018, fall protection training provided to employees was inadequate, in that, employees were allowed to take photos and measurements on the roof without fall protection after the actual start of construction work and before all construction work was completed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:  (a) On or about August 7, 2018, the portable ladder used to gain access to the roof, at the front of the house, did not extend at least 3 feet above the roof line.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Employees taking photos and measurements of the partially-completed roof while standing on the 6/12 sloped roof of a 1-story home are not being protected from the fall hazard by the use of a fall protection system.  The employer routinely fails to assure effective protection against falls as required by the standard, as demonstrated on August 7, 2018, when employees took photos and measurements on the roof of a one story residence at 487 W Artesia St, Oviedo, FL 32765 without fall protection and were exposed to a fall hazard from the roof.  Jasper Contractors Inc. was previously cited for a Willful violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1168383, citation number 2, item number 1 and was affirmed as a final order on 11/8/2017, with respect to a workplace located at 612 Robin Lane, Kissimmee, FL 34759.
