29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided:  a) Jobsite: The employer did not ensure that a ladder or stairway was provided at the jobsite as safe means of access to the entrance of the residential building approximately 3 feet above the ground surface.  Condition was observed on or about 9 AUG 2018.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) Jobsite: The employer did not ensure that the job made ladder was set with the side rails at least three feet above the landing surface exposing employees to fall hazards of approximately 10 feet.  Condition was observed on or about 6 AUG 2018.

Condition was observed on or about 9 AUG 2018.  Etna Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1147085, citation number 1, item number 2/a and was affirmed as a final order on 12 DEC 2016, with respect to a workplace located at 413-415 Wallace Street, Philadelphia.  Etna Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1164504, citation number 1, item number 2 and was affirmed as a final order on 17 JAN 2017, with respect to a workplace located at 1216 North 5th Street, Philadelphia.
