OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees where employees were exposed to crushing and struck-by hazards when using automotive lifts and jacks:  a) On or about August 16, 2018, and at times prior, the employer did not perform repairs to lifts following an inspection by a qualified lift inspector.  Deficiencies were noted on the following lifts:  1.  Bendpak lift,  Model PR-12F (Bay 1): Overhead shutoff not operational; reinstall chain link; passenger side arm locks in need of repair or replacement; no label set;  2. Bendpak lift (Bay 2): install overhead shutoff; install pads; indicate lift capacity;  3. Worth lift, Model 015309-0797, Ser. # 9000CF (Bay 3): Install overhead shutoff; Install arm locks;  4. Greg Smith Equipment lift, Model PRO-410, Ser# 5101270 (Bay 4): replace arm lock gear;  b) On or about August 16, 2018, and at times prior, the rolling lift jack on the Bendpak alignment rack (Bay 5) was used to lift a vehicle which exceeded the jack's rated capacity.
