OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees where employees were exposed to crushing and struck-by hazards when using automotive lifts.  a) On or about April 16, 2019, and at times prior, the employer did not perform repairs to lifts following inspection in 2018 by a qualified lift inspector and a separate  inspection by an OSHA compliance officer.  The following deficiencies were noted as indicated below:     1.  Bendpak lift Model PR-12F (Bay 1): The overhead limit switch was not installed and the label set was missing.     2.  Bendpak lift (Bay 2): The overhead limit switch was not installed, the metal support pads were missing, and the lift capacity was not indicated on the lift.     3.  Worth lift Model 015309-0797, Ser. # 9000CF (Bay 3): The overhead limit switch was not operational and arm locks were not installed.     4.  Greg Smith Equipment lift, Model PRO-410, Ser # 5101270 (Bay 4): An arm lock gear would not lock a lift arm in position.   The D & L Automotive Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard (OSH ACT of 1970 Section (5)(a)(1)), which was contained in OSHA inspection number 1339789, citation number 1, item number 1, and was affirmed as a final order on (October 31, 2018), with respect to a workplace located at 26050 Rd L, Cortez, CO 81321.
