29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On or about 8/15/2018 at a residential construction site located at 1312 Goshen Dr. in Lee's Summit, MO: Employees were exposed fall hazards. The employer had not developed, or implemented, a safety and health program. The employer failed to train their employees on the recognition of hazards associated with falls. This was demonstrated at the residential construction site when 4 employees were exposed to falls of approximately 10 feet while working from the framed structure of the new residential home. Employees had not been trained to recognize, evaluate, and control exposure to fall hazards.  b) On or about 8/15/2018 at a residential construction site located at 1312 Goshen Dr. in Lee's Summit, MO: Employees were exposed fall hazards. The employer had not developed, or implemented, a safety and health program. The employer failed to train their employees on the recognition of hazards associated with falls. This was demonstrated at the residential construction site when 8 employees were exposed to falls of approximately 10 feet while working from the main level of a new residential home, in which a floor opening was not covered or guarded. Employees had not been trained to recognize, evaluate, and control exposure to fall hazards.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  At a worksite located at 1312 Goshen Dr., Lee's Summit, MO: Employees were exposed to struck-by hazards. Employees were observed using pneumatic nail guns without the use of protective eyewear.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected prior to each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.    At a residential construction site located at 1312 Goshen Dr., Lee's Summit, MO: Employees were exposed to electrical shock hazards. The employer failed to protect employee from electrical shock in that equipment was being used with a visibly damaged extension cord and was missing a ground pin. This was demonstrated when a MAKITA 5007F brand circular saw was connected to an extension cord with damaged insulation exposing the internal components and had clearly not been inspected prior to use.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    At the jobsite located at 1312 Goshen Dr., Lee's Summit,  MO:  Employees engaged in residential framing activities were exposed to an electrical shock hazard. The employer failed to protect employees from electrical shock hazards by allowing the use of flexible cords and cables with no strain relief. This was demonstrated when a BOSCH RS325 brand reciprocating saw was connected to an extension cord and was found to have no strain relief at the end connecting to the extension cord.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  At a worksite located at 1312 Goshen Dr., Lee's Summit, MO: Employees were exposed to fall hazards. This was demonstrated when an opening from the main level of the new residential home did not contain any measure of fall protection such as guardrails, covers, or use of personal fall protection by employees working in the area. Employees had placed a 2x4 piece of wood to allow then to cross the opening rather than having to walk around.
