29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     21 Baldwin St, East Longmeadow, MA    On August 21, 2018 and at times prior, the employer had not developed or documented energy control procedures for all machines including the trunkline and product printer machines.  The maintenance employees did not utilize energy control procedures while performing work on other machines and band saw operators did not place a lockout device on the disconnect switch while changing the blades on the saws.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    21 Baldwin St, East Longmeadow, MA    On August 21, 2018 and at times prior, some energy control procedures were incomplete and non specific including the Rouselle Press Plenum Die machine.  The procedures for the die machine did not identify kinetic energy of the flywheel as a source of energy and did not identify which of the three electrical disconnect locations was to be used.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    21 Baldwin St, East Longmeadow, MA    As of August 21, 2018 and for years prior, the employer had not performed a periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task.  The employer also did not ensure that the inspector reviewed each authorized employee's responsibilities under each energy control procedure.
