29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: 21 Baldwin St, East Longmeadow, MA: As of December 22, 2020, the employer had not performed an annual periodic inspection in over two years that ensured each procedure was inspected for inaccuracies during an actual energy control task. The employer also did not ensure that the inspector reviewed each authorized employee's responsibilities under each energy control procedure they were authorized to use. The Duc-Pac Corporation was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1340447, citation number 1, item number 1c and was affirmed as a final order on October 29, 2018, with respect to a workplace located at 21 Baldwin St, East Longmeadow, MA.

29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On December 19, 2020 and at times prior, employees operated a foot-pedal activated press brake (PB-8) without sufficient guarding to prevent body part exposure in the point of operation. On December 19, 2020, an employee who was operating the PB-8 press brake received multiple finger amputation injuries. The Duc-Pac Corporation was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1340447, citation number 1, item number 3 and was affirmed as a final order on October 29, 2018, with respect to a workplace located at 21 Baldwin St, East Longmeadow, MA.
