29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:    On or about August 9, 2018 and times prior, the employer did not keep the following passageways, storage areas, and walking-working surfaces in the south and north plants in a clean and orderly condition: -  a. The Suspension Weld Line and Receiving Areas in the South Plant had broken pallets, metal baskets/crates, trash containers, premium weld wire boxes, and Miller welders that were not placed in an orderly manner in passageways and walking-working surface.   b. Area 10 pedestrian passageways in the South Plant were blocked with metal baskets/crates, wooden pallets, cardboard boxes containing parts, and forklifts, exposing employees to struck-by hazards from forklifts.  c. The storage passageways in the South and North Plants were blocked with pallets, boxes of goods, and bales of cardboards.

The eyewash/safety shower station south of the small part department in the South Plant was blocked by a Tennant 5680 Scrubber/sweeper and boxes of parts on a wet floor.  b. The eyewash/safety shower station by the battery charging station in the South Plant had no water pressure to flow and was later blocked by pallets of boxes.  c. The employer did not have a standard eyewash/safety shower at the North Plant and an emergency eyesaline wall station in the maintenance shop was used continuously.
