(a) 300 Main Street, Bradley Beach, NJ:  On or about August 21, 2018, Dumac Inc. employees were working in excavations pouring and finishing structural footings/foundations.  The spoils piles for the excavations were placed directly at the edges of the excavations, exposing the employees to potential hazards of objects or soil falling or rolling into the excavations.
