Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected: On or about: 8/21/2018 Location: Raw dough production area a) The employer did not require 'frequent' and 'periodic inspections' of the chain hoist. Employees who use the chain hoist lifting devices which have not been maintained and inspected are subject to struck by hazards from uncontrollable loads falling from failing devices.

One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: On or about: 8/21/2018 Location: Dough Production Area; Oshikiri Dough Divider DK-3C; S/N: 802271 a) The employer did not provide a special hand tool or guarding for retrieving dough inside a the dough divider hopper throat. An employee was exposed to a caught between hazard resulting in amputation of the right hand fingertips.
