29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:     On and before August 21, 2018, the slitter operator employees were not trained to the authorized level for lockout tagout. The employees conduct set up maintenance tasks within the Deacro paper slitter. The employees were changing and adjusting the circular metal blades while dependent on an interlocked control limit of the blade section barrier guard.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     On or about August 21, 2018, employees were exposed to machine hazards associated with moving parts, in-going nip points, and thermal hazards during a maintenance servicing task. There were two slitter operator employees that did not lockout the electrical and pneumatic energies while changing and adjusting circular metal blades in the slitter section of the Deacro slitter machine, model C610C-165, serial number 1895-662. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect and the pneumatic valve [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.     prepare for shutdown [(d)(1)]; - ensure authorized employee has knowledge of the type, magnitude, the hazard, and the method or means to control the energy;    shutdown the machine or equipment [(d)(2)];    machine isolation [(d)(3)]; physically locate and operate all energy isolating devices that are needed to control the energy to the machine or equipment in such a manner as to isolate the machine or equipment from the energy source(s);    LOTO device application [(d)(4)]; affix a lockout or tagout device to each energy isolating device;    dissipation of residual energy [(d)(5)(i)]; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy;    verification of isolation [(d)(6)]; verify that isolation and deenergization of the machine or equipment has been accomplished;

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about August 21, 2018, employees were exposed to unguarded rotating parts and ingoing nip points at the Deacro slitter machines model C610-165, serial number 1895-662 and model C610E-165, serial number 2345-868 and the Kidder slitter machine model SBM 25, serial number S1045495. On both the entry feed and exit sides of the slitter machines there are operator controls near the rotating rolls with ingoing nip points and point of operation hold down sections.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  On or about August 21, 2018, there was an electrical distribution panel next to the employee personnel door passage from the slitter department to the warehouse. The electrical distribution panel was blocked by multiple empty cardboard boxes.
