29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:     On and about October 25, 2019, maintenance employees were not trained to the authorized level for lockout tagout. The employees conduct maintenance, setup, and servicing tasks within the facility as needed. At the time of inspection the employees were readjusting the front end of the die on Machine #21 which required the control of the hazardous energies present on the equipment.     Sun America Converting LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 1340853 , citation number 1, item number 2A and was affirmed as a final order on November 13, 2018, with respect to a workplace located at 46 North Rock Hill Ave, Alliance, OH.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):       a) On or about October 25, 2019 employees were exposed to machine hazards associated with moving parts, in-going nip points, and gravitational hazards during a maintenance servicing task. There were two maintenance employees that did not lockout the electrical and pneumatic energies while readjusting the front end of the die on Machine #21. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect and the pneumatic valve [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.       prepare for shutdown [(d)(1)]; - ensure authorized employee has knowledge of the type, magnitude, the hazard, and the method or means to control the energy;      shutdown the machine or equipment [(d)(2)];      machine isolation [(d)(3)]; physically locate and operate all energy isolating devices that are needed to control the energy to the machine or equipment in such a manner as to isolate the machine or equipment from the energy source(s);      LOTO device application [(d)(4)]; affix a lockout or tagout device to each energy isolating device;      dissipation of residual energy [(d)(5)(i)]; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy;      verification of isolation [(d)(6)]; verify that isolation and deenergization of the machine or equipment has been accomplished;      b) On or about Septemeber 26, 2019  employees were exposed to machine hazards associated with moving parts and in-going nip points during a servicing task. There was an employee that did not lockout the electrical and pneumatic energies while reaching through a guard at the front end of the Tulip #2 machine to retrieve paper which was stuck in the machine. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect and the pneumatic valve [per the 1910.147(d)(4) requirements]. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about August 8, 2019 the employer failed to ensure that effective machine guarding was in place on the Kunlan die cutter (S4). Employees were exposed to the pinch point of the die clamping area. At the time the light curtains were placed to high above the hazard area allowing an employee's hands to reach the hazard area without tripping the safe guard mechanism and stopping the machine.     b) On or about October 25, 2019 the employer failed to ensure that effective machine guarding was in place on the wrap sheeter within the facility. The equipment had missing components and broken barrier guarding allowing employees to reach hazardous areas on moving machine parts and in-going nip points.     Sun America Converting LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1340853 , citation number 1, item number 2C and was affirmed as a final order on November 13, 2018, with respect to a workplace located at 46 North Rock Hill Ave, Alliance, OH.
