29 CFR 1926.1101(i)(2)(i):     The employer shall ensure that laundering of contaminated clothing is done so as to prevent the release of airborne asbestos in excess of the TWA or excursion limit prescribed in paragraph (c) of this section.      a) At the worksite; North Building, First Floor Locker Rooms, demolition and renovation project;  the employer did not  provide personal protective equipment, to employees with the job title of laborers performing Class 2 work; on or about 7/16/18, 7/17/18, and 7/18/18.      Note:     Because abatement of this violation is already documented in the case the employer need not submit certification or documentation of abatement of this violation normally required by 29 CFR 1903.19.

29 CFR 1926.1101(k)(3)(i): Before work in areas containing ACM and PACM is begun; employer did not identify the presence, location, and quantity of ACM, and/or PACM therein pursuant to paragraph (k)(1) of this section.    a) At the worksite; North Building, First Floor Locker Room demolition and renovation project;  the employer did not determine the presence, location, and quantity of ACM and/or PACM for employees with the job title of laborers; on or about 7/16/18, 7/17/18, and 7/18/18. 29 CFR 1926.1101(f)(2)(i): Each employer who has a workplace or work operation covered by this standard did not ensure that a "competent person" conducts an exposure assessment immediately before or at the initiation of the operation to ascertain expected exposures during that operation or workplace. The assessment was not completed in time to comply with requirements which are triggered by exposure data or the lack of a "negative exposure assessment," and to provide information necessary to assure that all control systems planned are appropriate for that operation and will work properly.    a) At the worksite; North Building, First Floor Locker Room demolition and renovation project;  the employer did not conduct an exposure assessment for employees with the job title of laborers; on or about 7/16/18, 7/17/18, and 7/18/18. 29 CFR 1926.1101(k)(9)(iii):     Training for Class I operations and for Class II operations that require the use of critical barriers (or equivalent isolation methods) and/or negative pressure enclosures under this section shall be the equivalent in curriculum, training method and length to the EPA Model Accreditation Plan (MAP) asbestos abatement workers training (40 CFR Part 763, subpart E, appendix C).      a) At the worksite; North Building, First Floor Locker Rooms, demolition and renovation project;  the employer did not  provide training, to employees with the job title of laborers performing Class 2 work; on or about 7/16/18, 7/17/18, and 7/18/18.    Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
