29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On the east side of the commercial building, employees engaged in roofing activities on a low slope roof without fall protection were exposed to fall hazards of approximately 24 feet to the ground below.

29 CFR 1926.602(c)(1)(vi): 29 CFR 1926.602(c)(1)(vi):Section 603E Industrial truck(s) American National Standards Institute, B56.1-1969, Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed. On the east side of the commercial building, the employer did not ensure that safe operating procedures or practices in accordance with section 6 of the ANSI B56-1-1969 or greater for JLG SkyTraks were followed exposing employees to stuck-by and crushed hazards: a) The JLG SkyTrak was unattended by more than 25 feet with a trash bend attached to the forks. b) Employees were not protected from standing or passing under the suspended load.
