29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 20 and times prior thereto, the sampling area conveyor was not guarded in a way to keep employees out of the area of operation when the pipes were kicked off the conveyor. This condition exposed employees to caught in hazards.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: On or about August 22, 2018 and times prior thereto, in the Small Diameter Plant in the pipe testing area, the cord attached to magnet used for cutting pipes was damaged and covered with electrical tape. This condition exposed employees to shock hazards.
