On September 7, 2018, the employer failed to provide OSHA Form 300 and 300-A records within four hours after the email request by the OSHA Compliance Safety and Health Officer following up on the August 27, 2018 on-site request.  Misamis Construction (Saipan) LTD was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.40(a), which was contained in OSHA inspection number 1119202, citation number 2, item number 1 and was affirmed as a final order on April 8, 2016, with respect to a workplace located at Zen Teppanyaki Restaurant, Coconut Street, Garapan Saipan 96950.
