29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  Building 3703, new walk-in freezer; on or about 22 August 2018 and times prior thereto; the employer did not ensure that walking and working surfaces were free of ice accumulation. The new Hussmann walk-in freezer, Model D-36-83, Serial number 27206 had significant amounts of ice accumulated on the floor creating slippery surface conditions. This condition exposed employees to a fall hazard.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Building 3703, delicatessen and bakery area; on or about 22 August 2018 and times prior thereto; a Hussmann frozen food walk-in freezer model D36-78, serial number 27207 emergency escape plunger was inoperative. Plunger would not cycle. This condition exposed employees to a hazardous indoor work environment.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  Building 3703, new walk-in freezer; on or about 22 August 2018 and times prior thereto; in the new Hussmann walk-in freezer, Model D-36-83, Serial number 27206 had a damaged two-place overhead electrical outlet. Outlet is used for the trace tape. This condition exposed employees to an electrical hazard.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  Building 3703, warehouse sorting area; on or about 22 August 2018; the employer did not ensure that the work areas were clean and orderly. Adjacent to the loading docks, damaged warehouse pallets were stored between the two loading docks impeding the walking and working surface. This condition exposed employees to a tripping hazard.
