29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: (a) On or about August 23, 2018 - Specialty Cell, employees were exposed to trip hazards when the metal grate that covers the trough that transports away the liquids that drain out of the cylinders was damaged at the edge of the station near the blue 55 gallon drum of Makerstrip 5000. The grate was dimpled and sank down when stepped on. (b) On or about August 23, 2018 - Specialty Cell, employees were exposed to trip hazards when the metal grate that covers the trough that transports away liquids that drain out of the cylinders was damaged at the computer station. The grates were uneven and sank down when stepped on.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: (a) On or about August 23, 2018 - Specialty Cell, employees were exposed to trip hazards when the grates over the drain pits that the employees walked on while testing and moving cylinders had not been inspected to discover that the grates near the 55 gallon drum of Makestrip 5000 were dimpled and sank when stepped on and the grates in front of the computer station were uneven and sank when stepped on.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) On or about August 23, 2018 - Specialty Cell, temporary employees were exposed to corrosive chemical hazards when they did not know that Makerstrip 5000 in the 55 gallon drum next to the Specialty cell was used when drying the cylinders. Employees thought that only water was used in the process. They did not know where to find Safety Data Sheets or the information that would have been on them.
