CMG Process Inc. dba Apex Engineered Products; north building; - On or about 8/29/18, and at times prior thereto, a horizontal band saw used to cut pipe did not have an adjustable guard covering the unused portion of the blade and the blade guide rolls.

CMG Process Inc. dba Apex Engineered Products; north building --- Ryobi Grinder --- On or about 8/29/18, and at times prior thereto, a bench grinder was being utilized to sharpen knives.  The bench grinder did not have any guarding around the wheel or spindle.  In addition, the grinder did not have a work rest or a tongue guard.

CMG Process Inc. dba Apex Engineered Products; south building; near pole S23 - On or about 8/29/18, and at times prior thereto, equipment including, but not limited to, an electrical box had two missing knockouts.  The box had a cord plugged into it that powered a light.

CMG Process Inc. dba Apex Engineered Products; south building; - On or about 8/29/18, and at times prior thereto, equipment including, but not limited to, a Lincoln Welder had a missing ground prong.  The welder was used daily to weekly to perform welding activities.
