OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard. On or about March 31, 2018, at 1200 University Ave, Monroe, LA, employees operated a Gravely Pro-Turn 400 riding lawnmower on inclines greater than 15 degrees near waterways creating the potential for a rollover event.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: On or about April 2, 2018, at 1200 University Ave, Monroe, LA, an employee was operating a riding lawn mower on a slope near a bayou edge, and the mower flipped over pinning the operator underneath the water in the bayou.
