Employees engaged in milling and cleaning activities on the 8th floor of the J.M. Smucker located at 2200 NW Brickyard Road in Topeka, Kansas were exposed to confined space hazards in that no signage was posted for the Remill Silo #9 hatch identifying it as a confined space.

Employees engaged in maintenance and cleaning activities at the J.M. Smucker located at 2200 NW Brickyard Road in Topeka, Kansas were exposed to entanglement hazards. The 1 5/8 in diameter rotating shaft of the dust bin motor located on the 8th floor protruded 2 5/8 inches without being guarded.
