29 CFR 1910.147(c)(4)(i): The employer failed to provide training to ensure that the purpose and function of the energy control program was understood by employees:      a) Dry Department: The Employer failed to train employees to ensure that the purpose and function of the energy control program was understood in that slurry sampling inside the Mixer/Cooker #16 tank was conducted without the tank's ribbon agitators being de-energized, and locked-out and/or tagged-out, as discovered on or about 05/08/2019.    b) Dry Department: The Employer failed to train employees to ensure that the purpose and function of the energy control program was understood in that the Mixer/Cooker #16 sampling port was used to make chemical additions to the tank without the tank's ribbon agitators being de-energized, and locked-out and/or tagged-out, prior to conducting this task, as discovered on or about 08/22/2019.    c) Dry Department: The Employer failed to train employees to ensure that the purpose and function of the energy control program was understood in that the inside of the Mixer/Cooker #16 tank was cleaned and sanitized without the tank's ribbon agitators being de-energized, and locked-out and/or tagged-out, prior to conducting this task, as discovered on or about 05/23/2019.    To abate this item, Respondent agrees to revise programs and procedures as necessary to ensure that any minor serving operation taken on Mixer/Cooker #16 is done so in full compliance with 29 CFR 1910.147. Once completed, Respondent agrees to retrain all Authorized, Affected, and Other employees on said procedures to ensure that the purpose and function are understood.
