29 CFR 1915.89(c)(1): The employer did not ensure that, before any authorized employee performed servicing when energization, startup, or the release of hazardous energy could occur, that all energy sources were identified and isolated, and the machinery, equipment, or system was rendered inoperative:   a)    USS Mesa Verde LPD-19: On or about August 29, 2018, authorized employees task to remove sections of pipes in the engine room did not ensure that the hazardous energy was released prior to cutting into the AFFF pipe as a result employees required medical treatment.       Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
