29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:  On or about 03/07/2022 and times prior thereto, located at General Dynamics - NASSCO Norfolk, 200 Ligon Street, Norfolk, Virginia 23510; At the X31 Inside Machine Shop, an employee suffered a left arm/chest struck-by injury from a radial drill press paddle bit while drilling holes in dry dock oak wood blocks, which resulted in the employee being in-patient hospitalized.  The event occurred on 03-07-22 at approximately 9:30 AM EST and the employer did not report the in-patient hospitalization to OSHA until 03/10/2022 at 2:00 PM EST.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
