OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to crush and struck-by hazards:    Facility - On or about 08/028/18 and at times prior thereto employees operated the green Mitsubishi, LPS type, LPG-powered forklift without a seat belt installed, and drivers were not restrained inside the vehicle.    PakGlobal LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard Section 5(a)(1) of the OSH Act, which was contained in OSHA inspection number 1273759, citation number 1, item number 1, issued on 09/21/2016 and affirmed as a final order on  12/11/2017, with respect to a workplace located at 185 Builders Parkway, Cornelia, GA 30531.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturer's prior written approval:    a) Facility - On or about 08/28/18 and at times prior thereto the employer modified the original green Mitsubishi LPS type, LPG-powered forklift without manufacturer's approval and utilized fork attachments to extend the length of the forks for the elevation of oversized loads.     b) Facility - On or about 08/28/18 and at times prior thereto the employer modified the original green Mitsubishi LPS type, LPG-powered forklift without manufacturer's approval, and removed the front rack exposing the hydraulic hoses to damage from contact with the load.     Or in the alternative:    29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, and were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts:    a) Facility - On or about 08/28/18 and at times prior thereto the employer modified the original green Mitsubishi LPS type, LPG-powered forklift without manufacturer's approval and utilized fork attachments to extend the length of the forks for the elevation of oversized loads.     b) Facility - On or about 06828/18 and at times prior thereto the employer modified the original green Mitsubishi LPS type, LPG-powered forklift without manufacturer's approval, and removed the front rack exposing the hydraulic hoses to damage from contact with the load.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a) Plant - On or about 08/28/18 and at times prior thereto the employer installed additional power outlets connected with flexible cables which run through the roof trusses, and were attached to the walls. The cables were left in permanent service and were not protected inside conducts.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
