29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about 08/30/18 and at times prior thereto employees operated the joiner. The feed area of the machine was not guarded.  b) On or about 08/30/18 and at times prior thereto employees operated the joiner. The discharge area of the machine was not guarded. (The original cover was removed and the conveyor chain was exposed).   c) On or about 08/30/18 and at times prior thereto employees operated the rip saw. The discharge area of the machine was not guarded. (The original cover was removed and the conveyor chain was exposed).   d) On or about 08/30/18 and at times prior thereto the Ritter router Model R20DC, S/N 1002, 240 V, was not guarded.  Or in the alternative:  29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) On or about 08/30/18 and at times prior thereto employees operated the joiner. 29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  a) Plant - On or about 08/30/18 and at times prior thereto employees operated the Ritter router Model R20DC, S/N 1002, 240 V.  The foot switch that activated the pneumatic clamp had a broken strain relief device. The cables and terminals were exposed to damage and loss of insulation. The electrical equipment was not readily accessible in case of service or emergency.     b) Plant - On or about 08/30/18 and at times prior thereto the work space for the panel boards mounted inside the electrical room was obstructed  by cable reels  and boxes with materials.
