29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    b. On or about 8/30/2018, at Britannia Building #2 of Forrest Glen, 3950 Loblolly Bay Dr. Naples, FL 34114, an employee was exposed to a fall hazard of approximately 30 feet to ground level while working on a roof without fall protection.       c. On or about 8/31/2018, at Britannia Building #2 of Forrest Glen, 3950 Loblolly Bay Dr. Naples, FL 34114, two employees were observed exposed to a fall hazard of approximately 30 feet while working on a roof without fall protection.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:    On or about 8/31/2018, at Britannia Buildings #2 and #3 of Forrest Glen, 3950 Loblolly Bay Dr. Naples, FL 34114, a competent person did not provide employees with training in the correct procedures for erecting and using personal fall arrest systems, exposing employees to a fall hazard of approximately 30 feet while performing roofing work of 3-story residential buildings.
