29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a) Jobsite:  Interior of the 4 Story residential building, 2nd floor stairway floor opening - Employees conducting framing operations in proximity to the 2nd level floor hole, measuring approximately 76" in length, 43" in width, and with a fall distance of 9' 2" (from 2nd level to the 1st level), were exposed to fall hazards without the use of personal fall arrest systems, covers, or guardrails system.    Observed on or about 8/29/18.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a) Jobsite: 4 story residential building, wall openings, all floors - Employees framing the inside edges of the unprotected wall openings, with an inside bottom edge measuring 26 inches above the walking/working surface, and with an outside bottom edge measuring approximately between 7 feet (1st floor) to 27 feet (4th floor) above ground level, were exposed to fall hazards without the use of fall protection system.    Observed on or about 8/29/18.
