29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:    On or about August 30, 2018, the employer did not ensure that employees wear cut and laceration resistant gloves while removing a jam of flexible foam at the K-II Fecken Kirfel model number 9982 horizontal slicing machine.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about August 30, 2018, the employer did not ensure periodic inspections of their lockout tagout procedures were conducted annually on machinery such as but not limited to the K-II Fechen Kirfel model number M9982 horizontal slicer machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     On or about August 30, 2018, employees were exposed to machine hazards associated with moving parts, in-going nip points, and thermal hazards during a maintenance servicing task. There were two operator employees that did not lockout the electrical and pneumatic energies while trying to run and measure a length of flexible foam sheeting material through the K-II Fecken Kirfel model M9982 horizontal slicing machine. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect and the pneumatic valve [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.     prepare for shutdown [(d)(1)]; - ensure authorized employee has knowledge of the type, magnitude, the hazard, and the method or means to control the energy;    shutdown the machine or equipment [(d)(2)];    machine isolation [(d)(3)]; physically locate and operate all energy isolating devices that are needed to control the energy to the machine or equipment in such a manner as to isolate the machine or equipment from the energy source(s);    LOTO device application [(d)(4)]; affix a lockout or tagout device to each energy isolating device;    dissipation of residual energy [(d)(5)(i)]; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy;    verification of isolation [(d)(6)]; verify that isolation and deenergization of the machine or equipment has been accomplished;
