29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about September 4, 2018, at the west edge of a residential structure under construction located at 6257 Fauna Terrace in Hobe Sound Florida, an employee was exposed to a fall hazard of approximately 10 feet - 6 inches when using an extension ladder to gain access to and from a walking/working surface while the ladder was not extended at least 3 feet past the upper landing surface and was not secured to prevent accidental displacement.
