29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about September 4, 2018, at the west edge of a residential structure under construction located at 6257 Fauna Terrace in Hobe Sound Florida, an employee was exposed to a fall hazard of approximately 10 feet - 6 inches when installing wooden forms for a concrete balcony while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about September 4, 2018, at the west edge of a residential structure under construction located at 6257 Fauna Terrace in Hobe Sound Florida, an employee was exposed to a fall hazard of approximately 10 feet - 6 inches when using an extension ladder to gain access to and from a walking/working surface while the ladder was not extended at least 3 feet past the upper landing surface and was not secured to prevent accidental displacement.
