29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 9/5/18, Roof of a commercial building under construction; 1) Two employees were exposed to fall hazards of approximately 14'-8" to 23.5' while installing and tying the concrete bars (beam work) on a roof of a commercial building under construction and not protected from falling. Diamante Concrete Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(1), which was contained in OSHA inspection number 1307212, citation number 1, item number 1 and was affirmed as a final order on 9/28/18, with respect to a workplace located at 1627 Yerba Buena Court, Lake Worth FL 33467.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided: On or about 9/5/2018; Location: Roof of a new commercial building under construction; 1) An employee installing concrete bars as part of a beam work was climbing on a cinder block wall with a break in elevation of approximately 40 inches, to gain access to the top of a roof and was exposed to fall hazard of approximately 14 feet 8 inches to 23.5 feet.
