29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) On August 30, 2018, employees were exposed to amputation and crushing hazards when setting new spoil boards on the ONSRUD Model 193G18W8 CNC Router. An employee received a partial finger amputation in that the employer did not ensure energy control procedures were documented and utilized to properly control the pneumatic energy before changing the spoil boards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a. On or about September 5, 2018, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to perform activities such as changing the spoil boards on the ONSRUD Model 193G18W8 CNC Router, and are exposed to amputation hazards from pinch points between the boards and table in that the equipment was not properly deenergized.
