OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to struck by and crushing hazards when operating shop crane to suspend loads without the benefit of a hook equipped with a latching mechanism. a) Braun Industries Inc. at the address of 1181 Grill Rd Van Wert, OH; On or about March 15, 2024, the hook of the Econo-o-Master Floor Crane, used to hoist/lift the rear of a truck chassis did not maintain a functional safety latch mechanism effectively closing the hook's throat. Employees operating in proximity of the crane/hoist to handle the chassis in this condition were exposed to crushing injuries associated with displaced loads.

29 CFR 1910.242(a):Hand and portable powered tools or equipment were not kept in safe condition: a) Braun Industries INC. at the address of 1181 Grill Rd Van Wert, OH; On or about March 15, 2024, the employer did not ensure that the OTC 5-ton pneumatic service jack was maintained and operated in a safe condition. Employees utilizing the service jack to raise the axle assembly of the freightliner model: M2 200 truck without the benefit of the saddle pad to provide adequate friction between the jack cap (saddle) and load, were exposed to struck by falling components of the suspension in the event displacement.

Employees utilizing the OTC 5-ton service jack to support the truck axle assembly on a Freightliner M2 200 truck without the benefit of securing the load to prevent displacement were exposed to crushing hazards.
