29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  a) Ground floor:  Employees used an electric powered chop saw and an electric powered hand drill to build formwork and stair rails. The tools were connected to extension cords, which were routed to outlet devices in the basement and were not provided with a ground fault circuit interrupter. On or about 09/05/18.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a) Ground floor, elevator shaft formwork:  An employee used one 2" by 4" wood plank supported by the elevator shaft formwork and a metal pole as a scaffold platform for elevation to build the formwork. The scaffold platform which the employee was on while working was not at least 18 inches wide. On or about 08/07/18.  The Link Group Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.451(b)(2), which was contained in OSHA inspection number 1141454, citation number 1, item number 2 and was affirmed as a final order on 05/25/16, with respect to a workplace located at 312 Fayette Street, Manlius, NY 13104.
