29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):  (a)    On or around September 5, 2018 the employer exposed workers to hazards of being buried, entrapped, and being struck by falling equipment or soil when it did not use an adequate protective system for the trench located at the Southbound side of Beltway 8 East Service Road at Spencer Hwy in Pasadena, Texas where employees were installing a pre-cast concrete manhole in an unprotected excavation with depth greater than 4 feet, an unprotected length of approximately 10 feet and 6' wide in class B soil.
